Observation

The staff member that witnessed an incident that needed to be reported admitted to intake and to the licensor during the interview that the incident had occured over a week before it was reported. The director of the program also stated to the licensor during the interview that she was aware that staff member had serious concerns and did not follow up to make sure a report was made.
